Premium gasoline has higher octane than regular gasoline does. For some cars, it makes a difference and the owner sees better gas mileage. For other cars, it doesn't matter as much. For example, my Saturn's owners manual says to use 87 (mid-grade) but in my area, 85 is available and cheaper so I use the 85 (regular) and I still get great mileage. However in California, you have no choice where the lowest grade is 87.
